I would go dark lipstick (like a blackish or dark purple) with a light-ish foundation for contrast. Dark eyes would be awesome too. I would have very dark eyeliner with light foundation and red lipstick ! no blusher etc. hope this will help :)
Eyes:
-If you wanted to try fake eyelashes, they have a really cool effect in the dark
-Black eyeliner
-Purple or grey eye shadow for the sexy look you mentioned
Lips:
-Black lipstick for the all-black look
-Silver or bluish-silver lipstick for a witchy look
Foundation:
-Either of those would be fine. I suggest just going pale.
Other:
-You could try a black, purple, silver, or green hair-color spray
-Black nails
-Fishnet tights?
-Whatever else you think will add to your costume

Definatly white foundation or the lightest you can get.
You want the brightest red lip stick you can find. Cover Girl should have what you want.
Then go for dark eye make up.
Go sexy! Maybe black lipstick (if you can pull it off) and dark eyes. Remember the mole too!
Get fake lashes add a little bit more makeup then normal. This should make for a sexy witch.
